What does a Pets Best declarations page look like?

How to find your Pets Best declarations page online
Once you’ve logged into your Pet's Best account, locate your top navigation bar and find the “My Pets” drop-down. In that dropdown, select “Documents and Forms.”
Choose the appropriate policy and click “Your Declarations Page.”
